Evolve Behavioral Health Services accepts insurance from participating health plans.

Before beginning services, I encourage you to contact your insurance provider to verify your mental health benefits, including coverage, deductibles, copayments, coinsurance, and any referral or authorization requirements.

Insurance coverage varies by plan, and verification of benefits does not guarantee payment.

Private Pay

Private-pay options are available for individuals who prefer to pay directly for services or whose insurance plan is not accepted. Current service rates for clinical assessments and individual therapy sessions are $150.00.

Sliding-Scale Fees

I understand that financial circumstances can sometimes make accessing mental health services challenging. Sliding-scale fees may be available to eligible clients based on individual financial circumstances and availability.
